E/M coding
Evaluation-and-management billing — the CPT subset most outpatient visits use.
E/M codes describe office-visit complexity and time. Since the 2021 / 2023 E/M reforms, the documentation requirements eased and the code level is driven primarily by medical decision-making or total time. Several ambient scribes now propose E/M levels from the conversation.
