BIRP note

Therapy note format: Behaviour, Intervention, Response, Plan.

BIRP โ€” *Behaviour, Intervention, Response, Plan* โ€” is a therapy note format common in community behavioural-health and substance-use treatment. It centres the structure on what the clinician *did* and how the client responded, rather than on the patient's narrative report.

Agencies billing Medicaid or running golden-thread compliance often standardise on BIRP. Scribes that lead with behavioural health usually offer it natively.
