DAP note

Therapy note format: Data, Assessment, Plan.

DAP — *Data, Assessment, Plan* — is a standard therapy and behavioural-health note format. *Data* captures what was observed or reported; *Assessment* is the clinical interpretation; *Plan* is the next step. It's a tighter format than SOAP because therapy visits don't usually need a separate Subjective/Objective split.

Therapy-specific scribes (e.g. Mentalyc, Upheal) lead with DAP and related formats; general scribes can usually be templated for it.
