Therapy note format: Goal, Intervention, Response, Plan.

GIRP — *Goal, Intervention, Response, Plan* — emphasises the treatment goal the session worked toward. Useful for outcomes-driven behavioural-health programs where progress against treatment-plan goals is a billing or compliance requirement.
